Carers Self-Assessment

This self-assessment is intended for those who care for someone with a serious illness, physical disability, learning disability, mental health problems or frailty as a result of aging.

Completing this self-assessment will help us to identify the support we can provide for you.

If you need to raise a self-assessment on behalf of the Cared For person themselves please complete this Self-Assessment form instead.

What will I need to complete the Self-Assessment?

You will be required to create an account, or log into an existing account, in order to complete and submit the self-assessment.

You will need to provide as much detail as possible about the situation you feel we could support with. Please provide:

What help you require
What you (or the person you are referring) is finding difficult
What is currently working well
What outcomes you (or the person you are referring) is looking to achieve

Doing this enables us to correctly prioritise your request. Without this information we may not be able to identify any support you may need and so you will need to contact us again.

How long will it take?

The self-assessment will take 10-20 minutes to complete depending on the amount of information you need to provide.

Please be aware that for security reasons your session will timeout after 20 minutes of inactivity.

Saving, Reviewing and Printing your Self-Assessment

Please save your self-assessment regularly using the 'Save for Later' button at the bottom of the screen. This self-assessment will then be available for 31 days in your list of 'My Saved Forms'.

Once you have submitted the self-assessment you will be able to see a copy in the ‘My Submitted Forms' section of the portal for 365 days; after this the self-assessment will no longer be visible.

To print the self-assessment before submitting, use the 'Print' button at the bottom of the screen.

To print the self-assessment after submitting, you can access it via 'My Submitted Forms'
